Planning for Care
- Patients treatment and care plan is decided by disease management group
- Patient care is continuous at all the stage of care
- Throughout journey of treatment patient is taken care by multi-disciplinary teams
- Care planning is done after thorough discussion with patient/ family. Any change in care plan is communicated to the patient family
- During all phases of care qualified individuals are identified as responsible for patient’s care
- Patient care is coordinated in all care settings within the organization
- Information about patient’s care and response to the treatment is shared among medical, nursing and other care providers
- Referral of patient to other specialty, patient transfer from one area to another area is done following hospital guideline and is documented properly
- Communication regarding patient’s plan of care is shared during change of staff shifts in clear and documented manner
- Time line for delivery of care is followed as per set guideline
